The Collaborative Care Training Hub is a partnership between Meadows Mental Health Policy Institute, West Health Institute, and Northwestern Medicine, developed as part of the Northwestern Medicine West Health Accelerator.

The Collaborative Care Model (CoCM) integrates behavioral health services into primary care, making care accessible for patients who might otherwise face barriers to treatment. Through our free, self-paced courses, healthcare and behavioral health professionals can gain skills and tools needed to put CoCM into practice.
