Collaborative Care Training Hub
Expert-led, self-paced online trainings designed to equip your care team with the skills to implement the Collaborative Care Model and improve patient outcomes.
The Collaborative Care Training Hub is a partnership between Meadows Mental Health Policy Institute, West Health Institute, and Northwestern Medicine, developed as part of the Northwestern Medicine West Health Accelerator. Together, we are advancing the integration of behavioral health into primary care through the evidence-based Collaborative Care Model (CoCM).
The Collaborative Care Model (CoCM) integrates behavioral health services into primary care, making care accessible for patients who might otherwise face barriers to treatment. Through our free, self-paced courses, healthcare and behavioral health professionals can gain skills and tools needed to put CoCM into practice. Earn continuing education credits while learning how to enhance care coordination, expand access to treatment, and deliver truly integrated, patient-centered care.
